Release and Waiver of Liability and Indemnity Form

Soaring Spirit Dance Studio requires that you submit a Release and Waiver of Liability and Indemnity Agreement form in order to participate in Soaring Spirit activities. If you have already registered for upcoming classes and/or activities, but have not yet submitted this form, please fill out and submit the form online or download the form, print it, fill it out, and send it to Soaring Spirit Dance Studio (contact info) or bring it to class.

Photo Release Form

Students of Soaring Spirit Dance Studio may have the opportunity to be photographed at various events. Soaring Spirit requests permission from parents to include these photos in Soaring Spirit publications or advertisements. If you are willing to grant permission, please fill out and submit the form online or download the form, print it, fill it out, and send it to Soaring Spirit Dance Studio (contact info) or bring it to class.
