Photo Release Form for Soaring Spirit Dance Studio

Students of Soaring Spirit Dance Studio may have the opportunity to be photographed at various events. With permission from parents these photos may be included for Soaring Spirit publications or advertisements. To ensure privacy, specific names of children will not be included. No financial compensation shall be given for use of any photographs. I acknowledge that Soaring Spirit Dance Studio may choose not to use my photo at this time, but may do so at its own discretion at a later date. Soaring Spirit reserves the right to discontinue use of photos without notice. I am 21 years of age and am competent to contract in my own name. I have read this release and I understand its contents. I consent to and authorize the use by Soaring Spirit Dance Studio, or anyone authorized by Soaring Spirit, of any and all photographs which have been taken of my child (or self for adult dancers) for the choices indicated below.
