Release of Liability Agreement

Physical activities such as dance, drama and musical theater carry a risk of injury to participants. I hereby release Soaring Spirit Dance Studio and/or The Music Place, Inc. of any liability whatsoever arising out of an injury, damage or loss which may be sustained by said person during the course of involvement with any and all Soaring Spirit Dance Studio activities. This includes activities held in the studio, in theaters during performances or rehearsals, and during any activity sponsored, arranged or facilitated by Soaring Spirit Dance Studio or The Music Place.

I have read and understood this waiver, and accept and assume such risks.
